We Pharmacists find ourselves caught between three major for profit public players (Insurance, PBM, Employer) each trying to maximizing their profits at the expense of the others profits. Walgreens was forced to take a stand mostly because they were the first of the big boys' contract that came up for renewal. Probably at some point we Pharmacists will find ourselves in the same position. IMO .. those who have agreed to be PIC.. are the ones with the most at risk... since the board looks to them for the legal operation of the Rx dept.
I recently wrote about this http://healthblog.steveariens.com/?p=421
I read about RPH's saying since the ESI/Walgreens split that their volume is up 40% and staffing levels remain unchanged.. how many additional errors are being done, under such circumstances? We see chronic patients being submissive to edicts from their insurer.. because they don't have the physical/mental reserves to fight them. Are we in the same place? We are functioning at 100%+ all the time at work... that we don't have the time/mental/physical reserve to "put up a fight". The people/entities doing the "fighting back" are doing it on the clock... not off the clock as we would have to do.
Work environment issues are seemingly not part of the concern for the BOP's.. only NC has a 12 hr work day limit and a incident last July with Kelly Hoots and CVS.. who was being scheduled to routinely work 14 hrs days.. to this point.. the board has not publically address this issue http://www.jimplagakis.com/?p=5044
75% of the BOP's have chain execs sitting on them... just look at last week... the DEA yanks the DEA license of two CVS stores... in Sansford FL... so far, I have not seen any published action by the FL BOP.
The Pharmacy Alliance (www.pharmacyalliance.com) in Jan sent letters to all 51 boards asking them to address certain work environment issues that are potentially affecting a Rx dept med error rate.
http://healthblog.steveariens.com/?p=457
To date, the collective response from the boards have been a YAWN !
IMO.. the PIC is the one with the biggest risk... they are legally responsible to the BOP for legal operation.. if the staff RPH cannot get response/action from corporate for things that are causing patient harm... the staff RPH's may have no choice but to start filing complaints with the BOP because the PIC is allowing the Rx dept to operate under conditions that cause med errors and patient harm. Bottom line... if no RPH is willing to agree to be PIC... there is no business plan for the corporate employer, or the PBM...
